Laboratory/Test Results and Imaging Studies

Our office will notify you by telephone of your laboratory and pathology test results done at the time of your office visit, per your discretion. It is your responsibility to update your personal information at the time of the visit. If important information needs to be addressed and you cannot be reached by telephone, we will send a certified letter to you. However, it is our policy that sensitive information (i.e. HIV, STD), abnormal results, and tests that need follow-up or discussion will require an appointment with the physician.

Late and Missed Appointments

In the event you are more than 15 minutes late for an appointment, we reserve the right to reschedule your appointment to avoid inconveniencing other patients. If your tardiness does not interfere with patient flow then you will be seen accordingly. If you need to cancel an appointment, please do so within 24 hours of the scheduled time slot. We may charge $25 for missing an appointment without giving 24-hour notice for cancellation.

Co-pays, Insurance and Balances

If your insurance requires a co-payment, it will be due at the time of service. We gladly accept cash, check, or credit card. Updated and current insurance and credit cards are requested at each time visit. If you have an outstanding balance after your insurance claim is processed, your credit card will be charged for the outstanding amount. You will receive a confirmation of the charge along with a medical statement explaining the reason for you remaining balance.
